This case involves a 73-year-old female patient who underwent a bilateral breast implant exchange with bilateral lowering partial capsulectomy. This lovely woman was wondering if she should keep implants in her 780s, but was concerned about losing the cleavage she had gotten used to. She absolutely did not want any visible scars or an extensive surgery to reposition her nipples (aka breast lift or mastopexy). Ultimately she consulted with Board-certified plastic surgery Dr. Matthew Isakson to see what her options were. They decided that they could reposition the implant pocket behind her breast tissue using a lowering capsulectomy rather than a breast lift to elevate the nipple and breast tissue around her implants. Both of her old silicone implants were ruptured but she did not have capsular contracture. With a well planned, well executed surgery and modern breast implants she got an even better result than she had hoped for! The before-and-after photos, taken three months post-operatively, document the physical outcomes of the procedure. Initially, the patient presented with previous implants that appeared elevated and tight due to an overlying waterfall (or snoopy-dog) deformity. The post-operative images reveal a noticeable adjustment in the positioning of the implants, which now exhibit a more natural placement on the chest wall. The skin around the surgical site appears smooth and without signs of distress or significant scarring. This procedure aimed at correcting the position and feel of the breast implants, as evidenced by the visible changes in implant alignment and contour. Due to the ruptured nature of her implants (and how old they were!) her old size remains unknown, but her new implants were Natrelle Inspira SCLP 280cc highly cohesive silicone implants.
